A Simple Contact Form
How to use Forminator to create a form

On your WordPress Dashboard -> Click on Forminator which is usually located on the left side of the screen.
Click on the type of form you would like to create. I will be creating a “contact us” form.
Click on the “form” option and then “Create”
The following screen will pop up(Below), select the option Contact us (Premade form), or create your own using the Blank Option.

After choosing a template, the next step is to “Enter a name” for the form.
Design your form in the manner of your choosing, and add fields depending on the outcome you want. For a simple contact us form we will be adding these fields:
- Name
- Address
- Phone Number
- Free-text

After adding all appropriate fields, and designing the form to your specification, it is now time to add it to your WordPress website page.
After completing the form, you will be given the short code for embedding the form into the page. Simply copy and paste this into the embed or short-code option.
